Some of the recent discussion about mage items made me sort of curious. How do people RP realism in regard to sleeping with some of the stat boosting items? I can see jewelry items and certain types of clothing being easy to ignore, but helms, staves, weapons, books, etc.. How do you keep the realism alive? Just questions for discussion, I'm not calling anyone out.

I myself never see any realism in the way NWN does it. I more like the way it's done in World of Warcraft. You're allowed two trinkets, a relic (or ranged weapon depending on class), an off-hand and weapon (or 1 two-handed weapon) and you don't have to be actively holding them to get their benefits.
So you can have your weapon sheathed and still get the bonus from it. Or you can not be holding your shield and still receive any bonuses from it. You, of course, cannot hit anything without drawing your weapon, but you wouldn't lose that +3 intelligence simply because you weren't holding your book up high for all to see or because you don't have your blade out still dripping blood.
I feel that as long as the items are NEAR you - still in your personal possession - they are still granting you their bonuses. Hell, even the book of Infinite Spells is written in PnP to be granting the owner infinite spells even if the book is thousands of miles away. It merely has to be on the same plane.
So... I don't undress my PC's because doing so removes all their bonuses when they rest. But I do pretend that they are undressed as I feel the bonuses can still be used if they are nearby.
Edit: this applies, I think to all extra items like weapons and books. Armour, jewelry, and helms are a different matter.

The books are fairly easy to RP. Wizards and clerics aren't sleeping to regain their spells, they are studying and praying respectively. I typically treat the +3 int book as Lustig's spellbook *pulls out spellbook, settles down to study* then hit the rest button. A cleric could treat the +3 wisdom book as devotional material that they glance at once in a while when praying.

Personally, I have to shift from resting to not just being, I went to sleep, Alora or even usually Aleasha has other things I commonly emote when they "rest" and most mages I know have done similar things.
In the case of Alora I usually have her sorting spell components or re-memorizing to store the arcane spells within her again as I often rp her magic more as a disciple in which not only is knowledge needed, but the mind and body must be made to be able to hold spells (in which one could easily explain rings or other items holding spells she uses as if extending the limits of her mind *which they do*). Aleasha who is a sorc usually just meditates. She sits down somewhere when she feels safe and draws the weave to her, regaining her attachment to it, and relaxing her hold on it, something like one would a muscle.
To pnp lore, this is not how it works, as a mage must rest for 8 hours and prepare there spells, rather than what is above, however nwn and the encounters on our server also are not designed as a pnp game necessarily would be ether, nor do most spells operate the same way for the most part(with good reason, shutter at the though of a mage dominating your character for RL weeks..) but this also means that a change needs to be made to how it is handled in game.
The most common weapon I see used however in game is the book of +3int or +3 wis (wis/cleric dependent) in which most I have seen most mages simply claim that they have a enchanted spellbook that not only keeps a copy of there master spellbook hidden wherever it is hidden, but that it is similar enchanted to the rings, and that it empowers them long enough to attain other magical spells to instead hold the said energy.
