What if anything is the metric, or barrier of enrtry for an RP concept that is way left field? Like, a character with CIPA, or a character with hyper-metabolism, or a character who's double jointed or hypermobile? What about someone with no fear response, or someone who rushes at fear stimuli instead of revulsing or retreating, or someone with multiple personalities as a maladaptive behavior to overcome trauma? some There's a whole host of really peculiar and fascinating medical conditions that I rarely see in any RP setting I'm curious if they'd be allowed to be explored here and if they'd be accepted or would they be ridiculed as some kind of god modding despite being a real thing that happens in life? CIPA, or lack of standard fear response as a mental disorder come to mind for example.

Your mileage may vary. If it seems like a super-power, it's too much. If it seems debilitating, it's probably going to make it impossible to be an adventurer. Ability scores should reflect the condition, of course.
As for lack of standard fear response, I'd argue that some people already RP that by not RPing fear at all.
There have been (and still are!) characters that have multiple personalities. They don't go around broadcasting that fact or the details of why.

So, essentially it's really just a matter of making things work. If a person can pull whatever oddity they want in terms of RP quirks ICly without breaking server rules, it's fair game?
