For example lets use Keetena. If you know her very well and you sense her near you, you can send a tell "I sense you. Is there anything recognizable to let me know it is you?" She may respond with "You smell roses and lavender... my normal fragrance (just making this up).

You don't have to ignore them and you really shouldn't ignore them. If you sensed them you sensed them. You either need to find out if you recognized them (takes both parties) or act like you don't recognize them.
In the above example lets say you know Keetena well but she responds with "No you don't have a clue it is me." It would be perfectly acceptable to click her hostile, and send this tell *draws arrow back and points bow in your direction* Show yourself or die!

DM Rich has a good point... when Keetena and Ranan were good "friends", she had this habit of sneaking up while invisible. Of course I could see her OOC on my monitor, but I didn't react until Keetena sent a tell, "you sense familar flowery smell and a wisp of air crosses your neck" or something to that effect. Even then Ranan would look around not sure what the source of smell or draft was and speak aloud... heh interestingly enough a couple of times there were others nearby who thought Ranan was losing his mind!
